To provide evidence of coverage and claims histories to a third party, AANA Insurance Services requires a signed release from the insured member dated within two years. The signed dated release must accompany each request.

In the event of a claim, please have the following information available:

• Date of incident
• Patient information (Name, Age, Gender)
• Facility Information (Name and Address)
• A Brief Description of the incident
